Collier Drug Stores has added a new online sign up form for folks interested in receiving the the COVID-19 vaccine.
Folks who have already signed up for vaccination from Collier Drug need not resubmit the form, as they are already on the waiting list.
From the post announcing the new form:
If you have already sent in an email adding yourself to our wait list, there is no need to fill out this form. This form is only for people who have not added themselves to our wait list as of today.
We will continue to work down the wait list in chronological order based on when people contacted us to be added to the list.
The form asks for a name and contact information, and information that will help determine what tier of vaccine priority an individual might be in. There are boxes to select for essential workers in a variety of industries, for example, and for people with pre-existing conditions.
The state is currently vaccinating individuals 65 and older, along with workers in health care and education.
